Dementia Care and Support Funded by Medicare

Divine Home Care Services is proud to participate in the GUIDE Program (Guiding an Improved Dementia Experience)—a Medicare-supported care model designed specifically for people living with dementia and their caregivers. Through our partnership with Vesper Healthcare, we are bringing this innovative, no-cost support program to families in Waco and surrounding Central Texas communities.

What Is the GUIDE Program?

The GUIDE Program is a new initiative from the Centers for Medicare & Medicaid Services (CMS) that helps individuals diagnosed with dementia receive coordinated, person-centered care at home. It also provides resources and relief for family caregivers, so they don’t have to carry the burden alone.

The program includes:

All services are covered under Traditional Medicare—with no out-of-pocket costs to the patient or family.

Who Is Eligible?

To qualify for GUIDE, participants must:

  • Have Traditional Medicareas their primary insurance
  • Have an official diagnosis of dementia(including Alzheimer’s)
  • Live at homeor in an independent living, assisted living, or memory care setting
  • NOTreside in a nursing home, skilled nursing facility, or long-term care institution
  • NOTbe enrolled in hospice at the time of participation

If you meet these criteria, our team can help confirm eligibility and guide you through enrollment.

Our Role in the GUIDE Program

Divine Home Care Services is honored to serve as a local care partner in the GUIDE Program, working closely with Vesper Healthcare—a Medicare-approved provider with expertise in dementia diagnosis and care coordination.

As part of this collaboration, Divine provides:

  • In-home visits to assess needs and track progress
  • Personal care and daily living assistance
  • Support for caregiver stress and burnout
  • Direct communication with Vesper’s clinical care team
  • Help navigating appointments, medications, and transitions

Together, we make sure that families are supported—not just clinically, but emotionally and practically.
